Select the Awards Category appropriate to the size and nature of your business. Register your intention to enter by completing the registration form. To be eligible, it must be received by no later than 5pm
30 June 2010. Enterprises must provide a submission of up to 2500
words and should also include supporting documentation such
as charts, tables and graphics. The closing date for
submissions is 5pm 15 July 2010 Awards Application Forms can be downloaded at

Applications and submissions must be signed and forwarded to the Customer Service Institute of
